After completing Enrollment Process Steps 1 & 2 and upon receiving approval to register, please print, complete and return the below items:

2. Request for Records

3. Responsible Use Policy

4. Immunization Records (done in person)

5. Student Birth Certificate (done in person)

6. IEP / 504 / Professional Testing Reports (done in person)

7. UPCA EAGLE CLUB

Upon UPCA receiving your above items and registration fee, the UPCA team will contact you to schedule your next step.
